Passport applications will be accepted at the City Clerk's Office between 8:15 am and 4:00 pm Monday through Thursday, excluding holidays.  Applications are not accepted on Fridays.

ITEMS REQUIRED TO OBTAIN PASSPORT BOOKS AND CARDS

1. Original Birth Certificate (with raised seal) or Certificate of Naturalization or a previous US passport

2. Original Marriage Certificate(s)/Divorce Decree(s) or a previous US passport

3. Picture ID - Adults - Driver's License / Florida State I.D.
    Children 16 years and over - School ID / Driver's License

4. Two (2) Passport Photos with a white background

5. Two (2) Checks:

• Check #1 made payable to U.S. Dept of State in the amount of $75.00 for an adult (16 & over) passport book and $60.00 for children under the age of 16.
  
• Check #2 made payable to the City of Tamarac in the amount of $25.00.

6. PASSPORT PROCESSING TIME: Normal processing time is less than 4 weeks.  If rush processing of a passport application is desired due to imminent travel departure, there is an additional fee of $60.00 for the U.S. Dept of State to expedite this processing.  Expedited processing time is within 3 weeks.

7. CHILDREN UNDER 16 YEARS OF AGE: It is required by law that both parents appear along with their child to execute an application for a child under age 16.  If this is not possible, the single applying parent must show proof of sole custody OR consent of the second parent (Form DS-3053) OR a death certificate. 

Evidence of child's relationship to parents/guardian must be provided. To establish the parental or custodial relationship between the parents or legal guardian(s) and the child, one of the following must be submitted with the application: Certified U.S. birth certificate with parents' names; Certified and foreign birth certificate with parent's names; Court order establishing custody; or Court order establishing guardianship and authority to apply for a passport. If the parent(s)/guardian(s) name is other than that on these documents, evidence of legal name change is required.

NOTE: Previous US passports ARE NOT acceptable as proof of relationship to the applying parent(s) or guardian(s).

8.  PASSPORT CARDS: The U.S. Passport Card may only  be used for international travel by land or sea  between the United States, Canada,  Mexico, the  Caribbean and Bermuda.  THE CARD MAY NOT BE USED TO TRAVEL BY AIR.  Customers must apply for a Passport Card using the New DS-11 Form.   The card will have the same validity period as a passport book: 10 years for an adult, five for children 15 and younger. 

Passport Card Fees: Adult (16 & over) $25 and Children (Under 16) $10 to the US Dept. of State.  There is also an execution fee of $25 due the City of Tamarac for both Adults and Children.

If applying for a Passport Book and Passport Card at the same time there is only one execution fee of $25.00.

IMPORTANT NOTICE: Although customers may apply for a US Passport Card beginning on 2/1/08, Passport Card issuance to customers is not expected until Spring 2008.  Therefore, expedite service for Passport Cards isnot available at this time.
